Plan review is required for public food service establishments that are:
- Newly built,
- Converted from another use,
- Remodeled, or
- Re-opened after being closed at least 18 months

DBPR Form HR 5022-049, is a content guide for public food service establishment operators to use to apply for an alternative operating procedure for minimum bare hand contact provided in the US Food and Drug Administration Food Code, Section 3-301.11. This form is meant to be a guide for proper application of an AOP, but is not required. The division will accept an AOP application in another format as long as all pertinent information is provided.

Commissary Notification – Commissary Notification PDF
DBPR Form HR-7022. A commissary is a commercial establishment which provides required services to the mobile food dispensing vehicle. All mobile food dispensing vehicles that are not self-sufficient must use a commissary. At a minimum, the commissary provides for acquisition of potable water and the disposal of wastewater. The division is unable to approve plans for hot dog carts or mobile food dispensing vehicles that are not self-sufficient without this form. DBPR Form HR-7034. This form must be submitted no later than 48 hours after a Temporary Commercial Kitchen commences operations in a location. An approved license application and satisfactory inspection are required prior to commencing operations (this form is not an application for license to operate).
HR-7037 Request for Exemption – HR-7037 Request for Exemption from Creating and Maintaining a Division Online Service Account
DBPR Form HR 7037, The Division of Hotels & Restaurants requires each applicant, and each licensee or licensed agent, to create and maintain a division online service account to manage their application or license. Any applicant, licensee or licensed agent may request an exemption from this requirement by submitting this form. The division will provide any person granted an exemption with instructions on how to submit applications, forms and fees in physical format. Please note that an email address is required.

HR-7033 Proof of Compliance with Miya’s Law Background Screenings – HR-7033 PDF
DBPR Form HR 7033, complete this form and present it to your inspector when requested during your inspection. The form must include a list of all employees with their date of hire and should be updated as changes in staff occur. HUD Apartment Exclusion – HR-7032 PDF
DBPR Form HR-7032, Attestation for Exclusion from Public Lodging Establishment License. Effective October 1, 2012, the division no longer inspects apartment buildings that are currently inspected by the U.S. Department of Housing and Urban Development (HUD) and are designated primarily as housing for persons at least 62 years old (elderly housing). This exclusion only applies to individual apartment buildings, not entire apartment complexes. The division still licenses and inspects any apartment building that is not inspected by HUD and designated as elderly housing.
To apply for this exclusion, complete this form and return to the address on the form. If you are applying for a new apartment license, please attach this form with your license application. If all of the buildings in the complex meet the above criteria, you may choose “Full Exclusion” and the division will not license or inspect your complex. If you do not have an existing license with the division, you do not need to complete the License Number information on the form.
Human Trafficking Information Sheet – PDF (please also see related Industry Bulletin 2019-01)
DBPR Form HR 5022-104, provides useful information on identification and reporting of human trafficking. Public lodgings, by their very nature, provide excellent cover to human trafficking operations. Because of this, it is extremely important that hotel and motel staff members are educated on the signs of such activity so it can be reported in the proper manner.

DBPR Form HR 5030-083, was created as a convenience for public lodging establishments to print and post in an area accessible to employees. Please be sure to print the sign on paper of at least 11in x 15in (minimum size requirement, per Statute).
Miya’s Law Key Log – PDF
Per Miya’s Law, Transient and Non-Transient Apartments must maintain a log accounting for the issuance and return of all keys for each dwelling unit. A sample Key Log was created as a convenience for public lodging establishments to print. Establishments may use this key log sample or one like it for compliance.
Human Trafficking Training Log– PDF
A sample Training Log for compliance with Human Trafficking Statues was created as a convenience for public lodging establishments to print. Each public lodging establishment must provide, to the Department of Business and Professional Regulation upon request, a log of employee signatures and dates of annual approved training received.
