| Bottle Club License Application Form ABT-6036 – For obtaining a bottle club license (and retail tobacco products permit, if applicable) for a for-profit, commercial establishment, where alcoholic beverages are not sold, wherein patrons bring their own alcoholic beverages onto the premise for the patron’s consumption. MAY NOT be issued to a bona-fide restaurant, hotel, or motel. MAY NOT hold any other alcoholic beverage license for such premises. | Caterer’s Alcoholic Beverage License Application Form ABT-6011 – Used by food caterers licensed under Chapter 509, Florida Statutes, to obtain a license for selling or serving alcoholic beverages for consumption on the premises of a catered event at which the licensee is also providing prepared food. DOES NOT allow for the storage of any alcoholic beverages. May operate in any Florida county and cater multiple, concurrent events. | Common Carrier (Passenger Carrier) License Application Form ABT-6020 – Used by operators of railroads or sleeping cars; or by operators of steamship lines, buses and bus lines, airplanes and airlines engaged in interstate or foreign commerce or plying between fixed terminals and upon fixed schedules in Florida; to obtain a license for purchasing alcoholic beverages and selling or serving to their passengers. | Passenger Vessel Permit Application Form ABT-6021 – Used by the operator of a passenger vessel which has a cabin-berth capacity of at least 75 passengers, and which is engaged exclusively in foreign commerce, to obtain a permit allowing consumption on-board the vessel during certain periods of time. Culinary Education Program License Application Form ABT-6025 – A culinary education program license (CEP) allows for sales of alcoholic beverages only for consumption on premises of the culinary education program, and, if the culinary education program provides catering services, for consumption on the licensed premises of the catered event at which the culinary education program is also providing food. |

Alcoholic Beverage Importer, Broker, Sales Agent, Exporter and Brand Registration Forms
| Importer, Broker or Sales Agent Application Form ABT-6008 – For obtaining a new license, transferring the ownership of an existing license, or making changes to an existing license as an importer, broker, or sales agent of alcoholic beverages. Importers may arrange for the storage of alcoholic beverages in Florida only in a state bonded warehouse. Brokers, or Sales Agents may not possess alcoholic beverages in Florida. Importers, Brokers and Sales Agents may arrange sales only to Florida-licensed manufacturers or distributors. | Exporters Registration Application Form ABT-6026 – Used by a person or entity in Florida to register as an exporter that arranges the sale of alcoholic beverages to persons or entities outside of the state or to a ship’s chandler or duty-free shop. | Application for Inactive Status or Waiver of Active Operation Requirements ABT-6027 – • Used to notify the Division of the inactive status of a quota alcoholic beverage license (Section 3A); • Elect a one-time, automatic waiver of the active operation requirements for a quota alcoholic beverage license not to exceed 12 months (Section 3B); or • Request a conditional waiver of the active operation requirements for a quota alcoholic beverage license not to exceed 12 months (Section 3C). This is necessary if the license is not in a location. |

| Application for Lien / Mortgagee’s Interest in Spirituous Alcoholic Beverage License ABT-6022 – Used to record liens, assumptions or assignments, or the renewal or extension of a lien pledged against a retailer’s liquor license. A lien must be filed within 90 days of the dated, executed security agreement and promissory note. | Application for Vehicle Permit ABT-6016 – (No longer required by Florida Statutes) Used by retail vendors of alcoholic beverages to obtain a permit for a vehicle that is owned or leased by the licensed retailer. The retailer may deliver alcoholic beverages, or transport purchases from a licensed distributor to their licensed location or licensed off-premise storage facility in their vehicle with a valid vehicle permit. Licensed distributor’s salesperson of wine or spirits, and common carriers, ARE NOT required to have vehicle permits to transport alcoholic beverages. |

| Application for Temporary Permits or Special Sales License ABT-6003 – May be used by bona-fide non-profit civic organizations to obtain temporary event permits that allow consumption-on-premise sales of alcoholic beverages. All profits must be retained by the bona-fide non-profit civic organization. | Quota License Drawing Entry Form ABT-6033 – This application/entry form is only valid during the Quota Drawing period. The division accepts entry forms for 45 days beginning the third Monday in August.
